Recently a Federal Government change has taken place.
Bill C-40 enacted June 22, 2007 provides a GST exemption for the counselling services supplied at Oakridge Counselling. If you received counselling between October 3, 2003 and October 30, 2007 you are eligible for a GST Rebate since the tax was “paid in error.”
Please contact Oakridge Counselling to receive a receipt of taxes paid and forward that along with the GST 189 form below to Revenue Canada to apply for your refund.
